Your insurance company may require a referral for a specialist visit. Before a referral may be authorized, your child must be seen by our physician or nurse practitioner to determine the necessity of such a visit. Once a referral has been deemed necessary, you will be given the names of one or more specialists who participate in your healthcare plan. Since everyone's availability is different, we ask that you make the appointment with the specialist at a time convenient for you.

Once you have received your specialist appointment date, please call our office immediately to request a written referral. We will need the date, specialist's name, and medical problem for which your child is to be seen.

We ask that you allow us a 7-10 day turn around time to complete the referral form. We want to be able to serve you in the best way possible; therefore, we are unable to honor requests for routine referrals in less than one week and we will be unable to fax referrals, except in emergency situations.

Of course, in an emergency or urgent situation, requests for referrals mde the day before or the day of your specialist appointment will be honored and faxed as needed.

Your routine written referral form must be picked up at our office unless you have provided us a self-addressed, stamped envelope.
